F.O.I.A. Process and Procedure CPS information that is available for public review/distribution can be requested under the Freedom of Information Act. The public can request information by writing a formal request to the CPS F.O.I.A. Officer at the following address: Elizabeth Calhoun Requests may also be faxed to: 773-553-1621. FOIA Questions? Please call 773-553-1626 All requests have a 7 – 14 day deadline for receipt of information. If more time is needed to respond, the F.O.I.A. Officer will inform the requestor of the additional time needed. A fee of $0.15 per page will be assessed to responses that are over 25 pages in length. The F.O.I.A. Officer will inform requestors of any fees before response.
