Chicago Public Schools - School-Based Dental Program



In 2005, the Illinois State Board of Education mandated that every child in grades Kindergarten, 2nd and 6th present proof a dental exam completed by a licensed dentist no later than May 15th of every school year (HB 752; 105 ILCS 5/27-8.1).

In order to assist parents in meeting this mandate, CPS has partnered with the City of Chicago’s Department of Public Health to offer this service to not only those children who are required to have an exam, but to all Pre-K-8th grade students. All necessary documentation is located below for download in both English and Spanish.
