The NCLB Transfer Application is available for students in grades 9-12 who attend a select group of CPS high schools. Schools that do not make Adequate Yearly Progress for at least two years in a row are designated as “School Improvement” schools under NCLB. NCLB states that students from these
high schools have the right to apply to transfer to a
high school that is “not in School Improvement.”
Application process
Step 1: Find your child's school. To apply for the NCLB transfer application your child must currently attend one of the
schools included within this list.
Step 2: Determine which schools you'd like to transfer your child to. Download the list of
transfer school options.
Step 3: Download and complete the
NCLB Transfer Application. Before filling out this application, please have the following information available:
- Your child’s name
- Your child’s CPS student ID number (which can be found on your child's progress report or recent test score reports)
- Your name
- Your phone number
- The name of the school in which your child is currently enrolled
- Your child’s grade level as of September 2009
You can select up to five choices from the list of schools on the application. Four charter schools at the bottom of the application have waiting lists that you also eligible to apply for. Placement on the waiting list does not guarantee a seat.
Step 4: If you are applying for more than one child, please also complete the
Sibling Relationship form.
Applications must be submitted by July 31. Once your application is complete, please mail or fax it to:
Chicago Public Schools
Attn: NCLB Choice Program
125 S. Clark Street, 11th Floor
Chicago, Illinois 60603-4042
Fax: 773-553-2421
Please note: Due to the limited number of seats, not all students who apply will be offered a seat in a selected school. If there are more applications than available spots, seats will be filled through a computerized lottery. CPS will notify you if your child has been selected for an available seat by August 6.
