In the state of Illinois, students must provide proof of required immunizations and health exams before October 15 of the current school year or they will face exclusion from school.
Medical clinics
There are many clinics available throughout the City of Chicago to assist you and your family with immunizations and health exams. Once you’re on the medical clinics website, enter your address to find a clinic near you.
School health centers
CPS collaborates with local medical providers to operate clinics within select schools throughout the District. These centers provide primary and preventive health care services to students while reducing lost school time, removing financial barriers to care, and promoting family involvement.
Most sites only see students, and student family members during school hours; however, some sites see neighboring school students and community residents.
School Health Centers provide services at low to no cost to students. All sites accept All Kids Health Insurance and most accept major insurance carriers. Students must have a parental consent on file in order to receive services.
