All students in kindergarten, 2nd, and 6th grade must have a dental exam completed by a licensed dentist prior to May 15 of the current school year. If the student fails to present proof by October 15 of the current school year, the school may hold the student’s report card.
In order to assist parents in meeting this mandate, CPS has partnered with the City of Chicago’s Department of Public Health to offer this service to not only those children who are required to have an exam, but to all pre-k-8th grade students.
Students that return the consent and authorization form will receive the following services at school: dental exam, dental cleaning, fluoride treatment and dental sealants as needed. All students requiring additional treatment will be referred to a dentist located within their school zip code. All students will receive a toothbrush for their participation.

Dentist information
Since dental contracts are not yet signed, we cannot tell you who your child's dentist will be. As soon as contracts are signed, you should expect to receive an email providing a list of schools and their assigned dentist. Contact information for all contracted dentists will also be provided and available online here.
Your dentist will contact you directly to schedule appointments. If you have not heard from your dentist by November 20, please contact the dentist directly. School assignments and dentist contact information will available here soon.
Dental service schedule
Each school will schedule services directly with their assigned dentists. Services typically take place the same time each year.
Appointments take approximately 15 minutes. Dentists are scheduled at each school depending on the number of students with signed parental consent/authorization forms and the ages of those students.
