Dental examinations and oral health program 

All students in kindergarten, 2nd, and 6th grade must have a dental exam completed by a licensed dentist prior to May 15 of the current school year. If the student fails to present proof of exam by May 15 of the current school year, the school may hold the student's report card.

 

In order to assist parents in meeting this mandate, CPS has partnered with the City of Chicago’s Department of Public Health to offer this service to not only those children who are required to have an exam, but to all Pre-K-8th grade students.

 

Students that return the consent and authorization form below will receive the following services at school: dental exam, dental cleaning, fluoride treatment and dental sealants (as needed). All students requiring additional treatment will be referred to a dentist located within their school zip code. All students will receive a toothbrush for their participation. 

Dental forms 

In order for your child to receive dental services at no cost to you, please complete the consent and authorization form, and ask your child's dentist to complete the State of Illinois form below. Both forms are available below in English and Spanish.

Download, print and then take this form to your child's dentist for him or her to complete. Return the completed form to your child's school as proof that he or she had a dental exam.

Dental service schedule  

Each school schedules services directly with their assigned dentists. Services typically take place during the same time each year. Student appointments take approximately 15 minutes during the school day.
