What It’s Based On
The transition plan is created in collaboration between the school and the Juvenile Justice Team.
Why This Matters
Transition plans contain key information that will help the student successfully transition into a CPS school including but not limited to behavior, attendance, grades, credits earned, strengths, challenges, and applicable justice system-related information. Transition plans aid in the smooth transition of grades and credits and support the development of a re-entry plan at the receiving school.
