Payroll

Please use these forms only if you are a former employee and request information from the Payroll Services Department. You may download the documents and fax completed forms to Payroll Services, 773-553-2662 or mail them to Payroll Services, 42 W. Madison Street, Chicago, Illinois 60602. If you have questions, please call Payroll Services at 773-553-4729.

Note: If you are an active employee, all of these forms can be accessed and completed electronically by logging into self service at HR4U. Paper requests will not be processed if you are an active employee.

Layoff Assistance

Laid off employees are eligible for employment services designed to help obtain reemployment as soon as possible. These include services such as job search and placement assistance, access to grant funds for education and training programs, support programs and facilities, career transition services, Labor Market Information (occupational information and economic trends) etc. A web page has been set up to provide information about these services and includes a message board with the latest information. Faith's Law Authorization for Sexual Misconduct-Related Information

Per the Illinois School Code, (105 ILCS 5/22-94) (Faith's Law trailer bill), CPS is required to respond to applicable prospective employers with relevant information related to pending or substantiated findings of sexual misconduct against any of its current or former CPS employees.

To authorize CPS to release this information, please complete the Disclosure Authorization form and submit it to faithslaw@cps.edu. In addition, if a prospective employer is interested in learning more about a current or former CPS employee's employment history, which may fall outside of the Faith's Law trailer bill, please also complete the CPS Reference Check form.

It is the policy of the Board to prohibit unlawful discrimination, harassment and retaliation on the basis of any classifications protected by the Constitution of the United States, the Constitution of the State of Illinois and applicable federal, state or local laws or ordinances, including but not limited to discrimination on the basis of race, color, sex (including pregnancy discrimination), gender identity/expression, age, religion, disability, national origin or sexual orientation.
